Complete this form (see below) to request help for an acute health issue. By submitting this request you are agreeing to pay an online consultation fee that depends on the level of complexity:
- $35-50 – simple, depending on complexity
- $55-75 – moderate, depending on complexity
- $95 – complex, requires phone discussion
- Off-hours/Weekends – there is an additional fee of $25 for requests submitted after 4:00 pm weekdays or on Sat/Sun
- Holiday – Acute Health Support may be not available on holidays
Once your request has been received and reviewed, an invoice will be sent to you via email. When you have paid the invoice, you will be sent an email stating when you can expect to receive my response. I will respond within 24 hours.
